Observation

Through the course of interviews and inspections it was determined that the provider: expelled a child that did not exhibit behavior that presents serious safety concern for self or others and the provider did not attempt to reduce or eliminate the safety concern through reasonable modifications did not review the expulsion policy with the parent or guardian and provide to the parent/guardian steps that were taken to avoid expulsion did not report to the department the expulsion of the child
